Home healthcare providers agree to provide a specific number of visits monthly and deliver specific care services to their patients in the home. Clients affirm that these visits and services were provided with a signature on a form.
Some fraudulent home healthcare providers ask their patients to sign multiple forms at the first visit and then fail to revisit during the rest of the month. These forms are then submitted for payment to Medicare or Medicaid agencies.
